Provider Agreement and Enrollment

A Provider Agreement for the PCA

A PCA is considered to be a participating Provider in health service programs administered by the Minnesota Department of Human Services.

As a Provider, a PCA must read and sign the Provider Agreement provided by the Minnesota Department of Human Services.

An Enrollment Application for the PCA

Because a PCA provides services to Minnesota Health Care Programs, (MHCP) recipients, each PCA must submit an enrollment application. Part of the enrollment process includes reading and signing the Provider Agreement described above.
